NFPA 80 Annual Fire Door Inspection

Complete 13-point visual inspection for fire-rated door assemblies. Required annually per NFPA 80, Standard for Fire Doors and Other Opening Protectives.

Door Assembly

No open holes or breaks exist in surfaces of either the door or frame§5.2.4.2(1)

Check both sides of door leaf and full perimeter of frame

Glazing, vision light frame, and glazing beads are intact and securely fastened§5.2.4.2(2)

Verify glass is fire-rated and labeled if present

The door, frame, hinges, hardware, and noncombustible threshold are secured, aligned, and in working order§5.2.4.2(3)

No parts are missing or broken§5.2.4.2(4)

Door clearances at the door edge to the frame, on the pull side, do not exceed listed clearances§5.2.4.2(5)

Max clearance: 1/8" at head and jambs; 3/4" at bottom (or 3/8" for rated floor)

The self-closing device is operational — door fully closes and latches§5.2.4.2(6)

The self-closing device is not disabled or blocked§5.2.4.2(7)

No wedges, doorstops, or hold-open devices unless connected to fire alarm

Hardware & Operation

If provided, the coordinator is operational§5.2.4.2(8)

Applies to pairs of doors — inactive leaf must close before active leaf

The latching hardware operates and secures the door when in the closed position§5.2.4.2(9)

If provided, auxiliary hardware items do not interfere with the operation of the door§5.2.4.2(10)

IMPORTANT

Fire door inspections must be performed annually by a knowledgeable individual and documented in writing. Records must be signed and dated, and kept for inspection by the AHJ. Deficiencies must be repaired or the door replaced without delay (NFPA 80 §5.2.4.4).
