3Energy Source Inventory — §1910.147(c)(4)

Check each energy type present. Specify disconnect location, valve number, or block location for each.

Stored / Residual Energy Notes (capacitors, springs, hydraulic pressure, gravity loads, thermal, steam)

4Isolation Sequence — §1910.147(d)

Each step must be completed in order before work may begin.

1.Notify all affected employees that LOTO is about to occur and why
2.Review machine-specific energy control procedure (ECP) for this equipment
3.If operating, shut down the equipment using its normal stopping procedure
4.Isolate the machine from ALL energy sources (open disconnects, close valves, block lines)
5.Apply lockout devices AND tag with the authorized employee's name
6.Release/restrain stored or residual energy (capacitors, springs, hydraulic pressure, gravity, steam, thermal, air)
7.Verify isolation: attempt to start the equipment using normal controls (then return to OFF), test with multimeter at zero energy

5Release Procedure — §1910.147(e)

Complete each step before removing locks and returning equipment to service.

1.All tools have been removed from the equipment
2.All employees are clear of the equipment and notified
3.Guards and safety devices have been reinstalled
4.Each lock/tag is removed ONLY by the authorized employee who applied it
5.Equipment is re-energized and tested before turning over to production
