Hot Work & Fire Watch — employer practical training and certification record

Part 3 — Certification

I certify that the worker named above was trained on this site’s hot work program, permit procedure and fire watch duties, and in the use of the portable fire extinguishers provided (IFC 2024 §3504.2.4; OSHA 1910.252(a)(2)(xiii)).

Keep with the worker’s training record. Under the IFC, permits and pre-hot-work checks are kept at least 48 hours after the work. This form does not replace the employer’s own program, and LifeSafetyWiki does not certify competence. Not an OSHA Outreach (10/30-hour) card.
